In case the ID card is lost or becomes unusable due to the fault of the personnel;
1) The relevant academic, administrative, permanent worker and contracted personnel shall apply to the personnel unit of the Faculty / School where they work with the Annex-2 Personnel Identity Card Request Form attached to the Directive and notify the situation.
2) If the ID card is lost, it is stated in a separate petition.
3) A picture taken in the last 6 months, a document showing the identity information and the identity card fee determined by the University Administrative Board every year to the relevant account number of the Department of Strategy and Development of our University, and a copy of the receipt is attached to the application.
4) The faculty/school sends a copy of the application to the Personnel Department.
5) The faculty/school also sends the Identity Application Form in Excel format containing the identity information of the personnel, a scanned copy of the receipt of the payment of the identity renewal fee and a scanned photograph in .jpeg format to the kimlik@beun.edu.tr e-mail address.
NOTE: In case of issuing an ID card for the first time, in case of a change of title or in case of a change in the identity information of the personnel (change of name, surname...etc.), the ID Application Form containing only the identity information of the personnel will be sent to the kimlik@beun.edu.tr mail address in Excel format.
